Background: Thyroid and parathyroid hormones are essential components of the metabolic system its regulation. Concurrent hyperthyroidism with hypoparathyroidism is an extremely rare finding not considered a common etiology brain calcifications seen on imaging. Brain can cause range neurologic symptoms, including movement disorders, cognitive impairment, seizures. Prompt recognition treatment to prevent or minimize development associated symptoms. Case Report: A 39-year-old female presented emergency department in unconscious state generalized weakness tonic-clonic seizures for 1 day. On clinical examination, she had jerky movements her upper limbs, Glasgow Coma Scale score was 4/15. Supporting hypoparathyroidism, low levels serum hormone, calcium, vitamin D high level phosphorus. Her magnesium normal. profile revealed hyperthyroidism. Noncontrast-enhanced computed tomography scan at midbrain showed multiple bilateral hyperintense areas basal ganglia thalami suggestive calcifications. The patient treated calcium supplements antithyroid agents that successfully resolved Conclusion: This case provides important documentation hypocalcemia as result differential diagnosis patients approach used our be managing cases where underlying challenging identify. highlights importance thorough evaluation individualized plan
